Have a look at my home page (http://www.discpro.org/~leland/) and follow 
the link to SNMP Scripts.  There is a script there called
maxstats.pl  (source code and sample output are on the page)
which shows this exact information.

(available, dead, administratively down, suspect, in-use, and software
revision).

Note - this script only works for max software version 6.0.0 and higher.
