(ASCEND) Max 4004 O/S Has Taken A Complete Dump

One of our Max4004's has decided to die (and I mean DEAD, won't even boot),
I need to know how if possible, to totaly clear the O/S out of it, and
reinstall it back to a state exactly as if it were new from the factory by
use of a jumper setting, or some other way. Here is what it is doing. When
you turn on the power to it, it will inflate the essential, inflate the
expendable, then goes to starting system and hangs there. It is connected to
9600baud console, and never goes to the system test. You can not get to it
by ip, nor does pressing any keys make it do anything. Yes the terminal
settings are correct ( I am sure of it ). This all started after attempting
to do a console ( esc. [ esc. - ) restricted load, through zmodem protocol,
which was aborted ( ^C ) with no file sent to it, I then exited from
diagnostics, went back to the VT-100 menus, changed the baud rate from 36400
to 9600, saved the config, got my terminal program talking to it again, and
shut it down, never to reboot again. Weird huh? I have never run into this
before (duh) and would like some expert advice on how to "shoot this thing
in the head" as the previous support person I talked to suggested. Any help
is appreciated.
