Never?  Not prototypical!  If we're going to be realistic about this
have a look at a few instances of disease infliction ...

Isaiah 10:16 (English-NIV) "Therefore, the Lord, the LORD Almighty, will
send a wasting disease upon his sturdy warriors; under his pomp a fire
will be kindled like a blazing flame."

A wasting disease would gradually drain constitution and/or hit points,
along with strength.  It would only affect fighters though.

2 Chronicles 21:18 (English-NIV) "After all this, the LORD afflicted
Jehoram with an incurable disease of the bowels."

That one is rather terminal.  The implementation would be a massive
drain on the "food" level of a character, along with various urgent
toilet stops and a trail of objects that nobody would want to pick up.

2 Chronicles 16:12 (English-NIV) "In the thirty-ninth year of his reign
Asa was afflicted with a disease in his feet. Though his disease was
severe, even in his illness he did not seek help from the LORD, but only
from the physicians."

Well, a disease of the feet is pretty plain.  Lower speed.  Increased
noise.  But it also suggests that a clerical character should pray to
fix a disease rather than buy curing services from non-aligned sources.

Deuteronomy 28:22 (English-NIV) "The LORD will strike you with wasting
disease, with fever and inflammation, with scorching heat and drought,
with blight and mildew, which will plague you until you perish."

Have we got a mildew-type monster?  Green slime?  Heat we have.

Okay, that ought to leave enough room for approximation with the current
crossfire archetypes.  Of course, there was a _lot_ of curing of
diseases going on, but the player has to _ask_, or he shall not receive.
