Just the obvious: First of all, have you disabled PnP on the card and set
it specifically for those IO/IRQ settings?  And is this the only net card
in the system?  Sometimes you need to specify settings at boot time if
it's not the only card.  Also, have you determined whether or not Linux
thinks those resources (I know, the MS term...) are available?  Run 'cat
/proc/interrupts' and 'cat /proc/ioports' to be sure...
