I mailed Maurice and he helped me out.  The script syntax in the comments
of it are something like:

./ip2host [cache_file] < infile > outfile


The brackets and the <> threw me for a loop.  

The syntax, without a cache DB, is:

./ip2host < /var/log/httpd/access_log > outfile


I was just dumb. :)
