If I remember correctly, it's due to a loss of connection while no
data was transmitting or something like that. The message "DCD lost" is
the same thing, but that one happends when data is being transmitted and
that the connection is closed. Both messages are for a connection
closed with an LCP close/terminate.
Exemples: disconnecting phone line from modem, noise,
disconnecting power from modem, too many fallbacks/forward, too many
retrains, a non low DTR hang-up, etc.
