You can do it in /etc/hosts.[deny,allow]; read the manual.
Since you're going to disable telnet and install ssh in its place (trust
me, you are :)), you can use ssh's own native ability to allow or deny
specific hosts and users.

Yes, but unless you have excellent physical security you don't want to do
it.
