You speak of simultaneous logins.  The best solution to this is to get
a RADIUS server that can prevent this - it takes more than just RADIUS,
you need additional code to be effective.  Cistron RADIUS is free and
does this very effectively.

On the commercial side you have Lucent's RADIUS ABM and Port Authority,
Radiator, RadiusNT, Steel-Belted RADIUS, etc.

You can also to this with 3rd party monitoring software, the best known
and widest used would be TSMON - commercial software.
