(ASCEND) what is a bad modem?

In diags, when you do an "avm" what makes a modem bad?

Here is an example of the results...

Modem 5:9, 35 calls, 5 bad, last 32 calls = ff9eff77 dir=3

Could someone explain to me what the last three fields mean?
It is obvious what the first two are, but I cannot find anything in the docs
about the others.
