Problems configuring an ISA Pnp Ethernet card

I am attempting to install my 3com 3c509b ISA Ethernet card.  The module
loads under Linux, but the card does not initialize.  Attached is a copy of
a pnpdump from my kernel and the output from isapnp in debug mode.  After
running isapn, the card still does not show up in lsmod.  Can anyone tell me
what to do so that my card will initialize?  I am running Caldera OpenLinux
2.2 with no major updates.
