(ASCEND) missing channel on E1

I have an E1 connected to our MAX4030 running 7.0.22, which never allows
more than 29 simultaneous connections.  The 30th caller gets a busy signal.
I have the line configured as ISDN, Net 5, G.703, and all my channels are
Switched (apart from the D channel, which I can't change).  The comms
provider (BT) has looked into it but can't see anything wrong, so do I have
the settings wrong somehow?  For what it's worth, line 16 is shown as "s"
and line 31 is always shown as "-" on the console.  Anyone have any ideas?
